This resource is designed to help teachers who want to create a student-centered classroom environment, develop ownership of the learning in students, and build a growth mindset.

Teacher Tips

To use these self assessment forms as formative assessment, first demonstrate for students how to apply the learning scale criteria. Next students will provide evidence to support the self-assessment by describing their performance on an activity or assessment for this standard. I allow students to attach a copy of a work sample, but also require them to describe specifically how that sample shows mastery of the standard. Finally, students should record their questions or notes about what is difficult for them about the standard.

When introducing the Action Plan section, I conference with small groups of students who are at the same level on the learning scale. We discuss options (learning center activities, online tutorials, workbook pages, etc.) for review and enrichment. They can choose from these to practice the levels they have mastered and the next level on the scale. We also brainstorm possible enrichment projects that they can complete independently or as a small group. After students become familiar with the self assessment process and the variety of learning activities available in our classroom, they can propose an action plan and conference with the teacher for approval.

Completed self assessment forms can be added to a student portfolio to document student progress.

Ideas for Easy Implementation

  • Begin by using self-assessment forms only for the key standards identified by your district or team as priorities.
  • Have students complete the form only when they believe they have mastered the standard (reached level 3 on the learning scale).
  • Use this to plan differentiation/curriculum compacting for advanced students.
